: Social determinants of health (SDoH) are conditions in a person’s life that directly impact their holistic outcomes. The services you deliver and methods for service delivery have a large capacity to directly impact a person’s SDoH - with that comes great responsibility. During the current age of COVID-19, SDOH become even more critical since we know that factors like where people live, relationships, and health conditions can affect a person’s vulnerability and recovery from the disease. This session provides a case study from a provider-led managed care organization for individuals with developmental disabilities whose coverage area was in the heart of the original outbreak in New York City, that utilizes many techniques, including telehealth, and data to identify opportunities to improve SDoH and increase quality and outcomes.
